Official academic transcripts are sent upon receipt of the student鈥檚 written request. Transcripts are only considered "official" when addressed and sent directly to the intended recipient.

Enrollment verification can be obtained for insurance, loans, scholarships, etc.

Replacement credential(s) and diploma cover may be replaced upon request.

Notary and Pennsylvania State endorsement of student鈥檚 diploma or transcript for international service or employment.

Request your academic record be re-evaluated for completion of degree requirements.

Former students wishing to return and complete additional coursework must submit this form.
